The FDA did not approve mitapivat for sickle cell disease by its original Nov. 1, 2026, target, instead pushing its decision to Feb. 1, 2027. The agency extended the Prescription Drug User Fee Act goal date by three months after classifying additional safety data from Agios Pharmaceuticals as a "major amendment" on Oct. 28. The request stemmed from an FDA inquiry on Oct. 9 into whether a Risk Evaluation and Mitigation Strategy would be needed to monitor hepatocellular injury risk, a concern that had previously delayed mitapivat's thalassemia indication in 2025.
Agios submitted roughly nine months of updated safety data on Oct. 19 in response. The company said its analysis found no cases of hepatocellular injury in sickle cell patients comparable in pattern or severity to those seen in thalassemia trials, and it has continued labeling discussions with the FDA while preparing for a commercial launch. Agios has described the drug as "launch-ready" pending the agency's final decision.
The delay, rather than an approval or rejection, meant the drug did not win approval by the Nov. 2 deadline this question tracked. The Predict Six community leaned toward "no," with 40% predicting approval, matching the eventual outcome.
